Opt in or apply for Membership to receive your medical book. Redeem voucher
1. Information on participation in this offer forms part of these terms and conditions of entry. Entry into this offer is deemed to be acceptance of these terms and conditions set out below (‘Terms and Conditions’).
2. The offer is being run by MDA National Insurance Pty Ltd ABN 56 058 271 417 AFS Licence Number 238073 (‘MDA National Insurance’) of Level 3, 88 Colin Street, West Perth WA 6005, Australia, phone 1800 011 255. MDA National Insurance is a wholly owned subsidiary of MDA National Limited ABN 67 055 801 771 (‘MDA National’) (together MDA National Insurance and MDA National, the ‘MDA National Group’).
3. Entry is open to ‘Eligible Entrants’. An Eligible Entrant is an individual who:
1. is a member of MDA National (‘Member’) and who;
• is a final year medical student or an intern, holding a policy with MDA National Insurance in the year ending 30 June 2023; and
• has submitted their details via the ‘opt in’ form to the Junior Doctor Keep On Program.
or
2. is a final year medical student or an intern and has applied for Membership with MDA National.
4. The offer commences (AEDT) on Thursday 4 November 2021 and ends at 5.00pm (AEDT) on Monday 31 January 2022. Eligible Entrants must have submitted their details via the ‘opt in’ form by this time or applied for Membership to redeem the offer and no extensions will be considered.
5. The offer is not open to employees of the MDA National Group, their immediate families and other affiliated parties.
6. The offer is an Elsevier voucher of 1 x 100% discount code for either the On Call: Principles & Protocols handbook, or The Junior Doctor Survival Guide. The offer is non-negotiable and may be redeemed in either the paperback, or e-book formats, subject to availability.
7. The offer is only available to the first 2,000 successful applicants.
8. Only 1 voucher per Eligible Entrant is redeemable.
9. Once the entrant has submitted their details to MDA National via the ‘opt in’ form or applied for Membership with MDA National, a unique Elsevier voucher will be emailed to the email address registered by the entrant on the opt in form or their Membership application within 3 business days. The MDA National Group does not take any responsibility for any delay in the delivery of the voucher or it being lost, stolen, damaged or destroyed in any way.
10. Vouchers must be redeemed by 5.00pm (AEDT) on Monday 28 February 2022. The entrant is responsible for complying with the terms of the voucher.
11. The entrant must, on request, be able to provide evidence demonstrating that they are an Eligible Entrant and have complied with these Terms and Conditions.
12. The offer must be taken as stated and no compensation will be payable if the entrant is unable to use the offer as stated. The offer is not transferable to another person or exchangeable for other goods and services and cannot be redeemed as cash or other than as stated in the voucher.
13. The MDA National Group and its respective officers and employees will not be liable for any loss, damage or personal injury whatsoever (including but not limited to direct, indirect or consequential loss) which is suffered or sustained in connection with this offer, the promotion of this offer or the use of any voucher, except for any liability which cannot be excluded by law. All entrants indemnify the MDA National Group against all liability, cost, loss or expense arising out of their participation in the offer.
14. The MDA National Group is not responsible or liable for any Eligible Entrant’s inability to participate in the offer whether due to an inability to opt in or apply for Membership, problems or technical failures of any kind, unavailability or inaccessibility of any service or website, electronic or human error or otherwise.
15. Entry details remain the property of the MDA National Group. Entrants consent to the MDA National Group using the contact details provided in connection with their Membership and MDA National Insurance policy for the purposes of facilitating the conduct of the offer and awarding any vouchers.
16. The MDA National Group collects, uses and discloses personal information of entrants in accordance with its Privacy Policy for the purposes of conducting this offer. Entrants may access, change and/or update their personal information by contacting the MDA National Group on the number below. 17. Where this offer is communicated and/or conducted via a social media platform (including, but not limited to, Facebook, LinkedIn and/or Twitter), entrants acknowledge that use of social media platforms are subject to their respective terms and conditions of usage. The offer is in no way sponsored, endorsed, administered, nor associated with any social media platform.
