Please contact us about any matter that could result in a complaint, claim, investigation (e.g. Coroner, Medicare), employment dispute or disciplinary matter as soon as

possible by calling 1800 011 255 or completing our Notification of Incident Form.

Contacting us early enables us to provide you with the most appropriate support and assistance.
