NSW RANZCO & Ophthalmology Updates! ASM 2022 MDA National Prize Terms and Conditions

  1. Information on how to enter this competition forms part of these terms and conditions of entry. Entry into this competition is deemed to be acceptance of these terms and conditions set out below ("Terms and Conditions").
  2. The competition is being run by MDA National Insurance Pty Ltd ABN 56 058 271 417 AFS Licence Number 238073 ('MDA National Insurance') of Level 3, 88 Colin Street, West Perth WA 6005, Australia, phone 1800 011 255. MDA National Insurance is a wholly owned subsidiary of MDA National Limited ABN 67 055 801 771 (together MDA National).
  3. Entry is open to medical professionals who scan the QR code at the MDA National stand and submit an online contact details form during the NSW RANZCO & Ophthalmology Updates! ASM 2022, held at the Fullerton Hotel Sydney on Saturday 27 August to Sunday 28  August 2022. ('Eligible Entrant').
  4. The competition commences at 8:00am (AEST) on 27 August 2022 and ends at 3:45pm (AEST) on 28 August 2022.
  5. The prize consists of 2x RM William gift cards valued at $500 each.
  6. The prize will be drawn at 10:00 (AEST) on Monday 29 August at MDA National Sydney Office, Level5 69 Christie Street ST LEONARDS NSW 2065
  7. The result is final, and no correspondence will be entered into. If the winner is not present at the draw, the winner will be contacted by email within five (5) days of the draw date to arrange delivery of their prize.
  8. The prize winner must, on request by MDA National Insurance, be able to provide evidence which, to the reasonable satisfaction of MDA National Insurance, demonstrates that the winner is an Eligible Entrant and has complied with these Terms and Conditions.
  9. The winner will be given 2 weeks to claim their prize. If in this time the prize is not claimed, then a redraw will take place at Level 5 69 Christie Street ST LEONARDS NSW 2065 at 10:00am (AEST) on Monday 12 September 2022 with the winner notified in writing.
  10. The prize will be deemed unclaimed if MDA National’s reasonable attempts to contact the winner via email and telephone during normal work hours of Monday to Friday 8.30am – 5.00pm (AEST) are unsuccessful. Redraws will follow the same process until a successful claim is made.
  11. The prize must be taken as stated and no compensation will be payable if the winner is unable to use the prize as stated. The prize is not transferable to another person (unless agreed by MDA National Insurance) or exchangeable for other goods and services and cannot be redeemed as cash.
  12. MDA National and its respective officers and employees will not be liable for any loss, damage, or personal injury whatsoever (including but not limited to direct, indirect, or consequential loss) which is suffered or sustained in connection with this competition, the promotion of this competition or the use of any prize, except for any liability which cannot be excluded by law.
  13. MDA National is not responsible for any late, lost, incorrectly submitted, delayed, ineligible, incomplete or misdirected entry whether due to error or otherwise.
  14. Entry details remain the property of MDA National. The name of the winner may be used for promotional purposes by MDA National unless the winner otherwise notifies MDA National at the time of accepting his/her prize. Eligible Entrants consent to MDA National Insurance using the contact details provided in connection with their MDA National Insurance policy for the purposes of facilitating the conduct of the competition and awarding any prizes.
  15. MDA National collects, uses and discloses personal information of entrants in accordance with its Privacy Policy for the purposes of conducting this competition.   16. Where this competition is communicated and/or conducted via a social media platform (including, but not limited to, Facebook, LinkedIn and/or Twitter), entrants acknowledge that use of social media platforms are subject to their terms and conditions of use.
