Open Disclosure: Practical steps for everyday clinicians

Adverse events, near misses or medical errors have the potential to impact the patient, carers, staff, and the organisation. Managing these events and implementing the Open Disclosure process can be challenging. Understanding the importance of an apology and the steps to ensure patients and staff are supported through an Open Disclosure process is critical to ongoing patient care and successful health outcomes.

 

The workshop explores many aspects of the Open Disclosure process including:

  • Identify the Australian Open Disclosure framework and relevant legislation
  • Summarise the benefits and barriers to Open Disclosure
  • Identify when open disclosure/clinician disclosure is required
  • Describe the role and delivery of an apology in open disclosure
  • Determine what to say and do in an Open Disclosure conversation
  • Discuss the needs of patients, carers, family, and staff in the Open Disclosure Process
  • Discuss the importance of documentation and follow-up after the Open Disclosure process
  • Reflect on current work practices and areas of quality improvement
