Reimbursement Offer Terms and Conditions
Renew your Policy with MDA National and receive a $250 reimbursement for any Practice & Personal Growth courses you enrol in, in your first three years of private practice.1. Information on how to participate in this offer forms part of these terms and conditions. Acceptance of this offer is deemed to be acceptance of these terms and conditions set out below (‘Terms and Conditions’).
2. The offer is being made by MDA National Insurance Pty Ltd ABN 56 058 271 417 AFS Licence Number 238073 (‘MDA National Insurance’) of Level 3, 88 Colin Street, West Perth WA 6005, Australia, phone 1800 011 255. MDA National Insurance is a wholly owned subsidiary of MDA National Limited ABN 67 055 801 771 (‘MDA National’) (together MDA National Insurance and MDA National, the ‘MDA National Group’).
3. The offer is open to ‘Eligible Offerees’. An Eligible Offeree is an individual who:
- is a member of MDA National (‘Member’); and
- is a doctor in training in post graduate year four or five; and
- has renewed their 2021/22 Professional Indemnity Insurance Policy with MDA National Insurance (‘Policy’) or taken out a 2021/22 Policy for the first time; and
- already has in place a direct debit arrangement, or sets up a direct debit arrangement with MDA National Insurance.
4. The offer is a reimbursement to the value of $250 against the cost of any one or more Practice and Personal Growth courses which the Eligible Offeree enrols in during their first three years of private practice. Practice and Personal Growth courses must be recognised by an Australian medical education institute.
5. The reimbursement for any such course must be claimed within six months of completing the relevant course, and prior to the final closing date for all reimbursements able to be claimed under this offer, being [31 December 2030].
6. At the time of claiming the reimbursement, the Eligible Offeree must be:
- a Member and hold a current Policy in one of MDA National's private practice categories; and
- be enrolled in their first three years of private practice.
7. The reimbursement is not open to employees of the MDA National Group, their immediate families and affiliated other parties.
8. Only 1 reimbursement to the value of $250 per Eligible Offeree is redeemable.
9. On completing any relevant course, the reimbursement may be claimed by providing the course registration receipt and/or invoice, as well as evidence of completion of the course directly to MDA National Members Services Team via peaceofmind@mdanational.com.au.
10. Reimbursement will be provided by bank transfer or cheque, at the discretion of MDA National Insurance.
11. The offeree must, on request by MDA National Insurance, be able to provide evidence which, to the reasonable satisfaction of MDA National Insurance, demonstrates that they are an Eligible Offeree and have complied with these Terms and Conditions.
12. The offer must be taken as stated and no compensation will be payable if the Eligible Offeree is unable to use the reimbursement as stated. The offer is not transferable to another person or exchangeable for other goods and services and cannot be redeemed as cash.
13. The MDA National Group and its respective officers and employees will not be liable for any loss, damage or personal injury whatsoever (including but not limited to direct, indirect or consequential loss) which is suffered or sustained in connection with this offer, the promotion of this reimbursement offer or the use of any reimbursement, except for any liability which cannot be excluded by law. All Eligible Offerees indemnify the MDA National Group against all liability, cost, loss or expense arising out of their participation in the offer.
14. Details provided by Eligible Offerees in claiming the reimbursement remain the property of the MDA National Group. Eligible Offerees consent to the MDA National Group using any details provided in connection with their Membership or Policy for the purposes of facilitating the conduct of the offer and providing any reimbursement.
15. The MDA National Group collects, holds, uses, discloses and manages personal information of Eligible Offerees in accordance with its Privacy Policy for the purposes of conducting this offer. Eligible Offerees may access, change and/or update their personal information by contacting the MDA National Group on the number below.
