Live Well Work Well Retreat 2019 Competition – New South Wales Terms and Conditions

  1. Information on how to enter this competition forms part of these terms and conditions of entry. Entry into this competition is deemed to be acceptance of these terms and conditions.
  2. The competition is being run by the MDA National Insurance Pty Ltd (‘MDA National Insurance’) ABN 56 058 271 417 of Level 3, 88 Colin Street, West Perth WA 6005, Australia, phone 1800 011 255. MDA National Insurance is a wholly owned subsidiary of MDA National Limited ABN 67 055 801 771 (“MDA National”) (together the ‘MDA National Group’).
  3. Entry is open to New South Wales based Doctors in Training (FYS/Intern, PGY2 & PGY3)  who enter, update or confirm their details on the MDA National update your details page. Eligible entrants must be an MDA National member. Non-members wishing to enter the competition need to join MDA National prior to competition closing date following the link provided – this will form their entry. (‘Eligible Entrant’).
  4. The competition commences at 12.00pm (AEDT) Friday 26 October 2018 and ends at 5.00pm (AEDT) on Monday 31 December 2018.
  5.  The prize consists of one (1) of four (4) spaces at the MDA National Live Well Work Well Retreat 2019 valued up to an estimated $1422 (total prize pool of $5,688). Package includes: a place at the MDA National Live Well Work Well Retreat taking place from Friday 22 March until Sunday 24 March 2019 inclusive; return flights from your nearest major airport to Melbourne Tullamarine domestic airport; return transfers for one person from Melbourne Airport to Lancemore Hill at Macedon Ranges Conference Centre; two nights’ accommodation (single occupancy) including breakfast, two lunches and two dinners.
  6. The prize will be drawn at 1.00pm (AEDT) on Wednesday 9 January 2019 at Level 3, 100 Dorcas Street, Southbank Vic 3006.
  7. The result is final and no correspondence will be entered into. The winners will be contacted by email within five (5) days of the draw date to arrange delivery of their prize. The winners will also be published on the MDA National Facebook Page as well as on mdanational.com.au.
  8. Each prize winner must, on request by MDA National Insurance, be able to provide evidence which, to the reasonable satisfaction of MDA National Insurance, demonstrates that the winner is an Eligible Entrant and has complied with these Terms and Conditions.
  9. Each winner will be given 1 month to claim their prize. If in this time the prize is not claimed then a redraw will take place at Level 3, 100 Dorcas street, Southbank Vic 3006.at 1.00pm (AEDT) on Wednesday 6 February 2019 with the winner notified in writing and published on mdanational.com.au.
  10. The prize will be deemed unclaimed if MDA National’s reasonable attempts to contact the winner via email and telephone during normal work hours of Monday to Friday 8.30am – 5.00pm (AEDT) are unsuccessful. Redraws will follow the same process until a successful claim is made.
  11. The prize must be taken as stated and no compensation will be payable if a winner is unable to use the prize as stated. The prize is not transferable to another person (unless agreed by MDA National Insurance) or exchangeable for other goods and services and cannot be redeemed as cash or other than as stated in the voucher.
  12. The MDA National Group and its respective officers and employees will not be liable for any loss, damage or personal injury whatsoever (including but not limited to direct, indirect or consequential loss) which is suffered or sustained in connection with this competition, the promotion of this competition or the use of any prize, except for any liability which cannot be excluded by law.
  13. The MDA National Group is not responsible for any late, lost, incorrectly submitted, delayed, ineligible, incomplete or misdirected entry whether due to error or otherwise.
  14. Entry details remain the property of MDA National Insurance. The names of the winners may be used for promotional purposes by the MDA National Group, unless a winner otherwise notifies the MDA National Group at the time of accepting his/her prize. Eligible Entrants consent to MDA National Insurance using the contact details provided in connection with their MDA National Insurance policy for the purposes of facilitating the conduct of the competition and awarding any prizes.
